Step 1: Download the "Sign-up application" and the "Service Terms and conditions agreement" for your appropriate area.
Step 2: Print both forms.
Step 3: Fill out and sign the "Sign-up application".  
Step 4: Read and sign the "Service Terms and conditions agreement".
Step 5: FAX both the application and the agreement back to your appropriate area.  FAX numbers are listed below.
